Hello, and welcome to eApply!  This is your one-stop-shop for all eApply needs.  So if you:

♦  need an account created for a provider, complete the form on our New Account Request page.  A Cenevia staff member will reply with account credentials within one business day.

♦  are a new user (or just need a refresher!) please visit the eApply Information Guide for instructions on how to complete the eApply digital application process.  This page has everything from how to reset your password to the correct way to upload documents, so be sure to check it out if you’re new to the system.

♦  are working on your application and can’t find the correct item for selection (such as a language or hospital) please complete the form on our Add Item Request page.

♦  have any questions, or to report an error with the system, please contact an administrator.
